If you are registering for a 2012/13 Spring/Summer team, you must download and complete Form #1628. DO NOT fill out the form online.
If you are registering for the 2013/14 season, you must complete form #1650. You may submit this form online. Please read the instructions below, and click the "I Agree" button to begin.

Instructions
In order to complete the Online Team Official Registration & Risk Management Disclosure, you will need to provide one form of identification. Identification can be in the form of:
- Driver License or Identification Card
- Passport
- Social Security Number (optional)
- Military I.D.
- School I.D.
Upon completing the online form, please print your confirmation page and provide it to Capital Live Scan. Capital Live Scan will not complete your fingerprints without a completed Adult Disclosure Form.
